(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that two substitute staff members were missing Mandated Reporter Training for Child Care Providers.

Based on observation, the licensee did not comply with the section cited above. The playground structure in the play area has plastic that has peeled off, leaving sharp edges, and exposed metal which poses a potential health, safety or personal rights risk to persons in care.

Based on interviews and video footage, child #1 walked outside of the gated preschool area and was without adult supervision for approximately ninety (90) seconds. This poses an immediate health, safety, or personal rights risk to children in care.
