Based on LPA observation, the flooring at the entrance of the classroom is warped, lifted and discolored. This poses a potential risk to the health, safety and personal rights of the children in care.

Based on staff interview, Staff 2 and 3 did not have immunization records on file for pertussis, measles, influenza (or decllination). This poses a potential risk to the health, safety and personal rights to the children in care.

Based on record review, the licensee did not comply with the section cited above in that Site Supervisor's CPR/1st Aid certification expired in January 2022, which poses a potential health, safety or personal rights risk to persons in care.
