It was observed by LPA Ocegueda and Site Supervisor that there was a crack that caused lifting on the cement and breakage in a different area of the cement that made a hole. This could cause tripping of children in care. This poses a potential health, safety or personal rights risk to persons in care.

During employee records review, LPA Herrera observed that 2 out of 4 staff had an expired Mandated Reporter Training on file, which poses a potential health, safety or personal rights risk to persons in care.
