During the inspection LPA Herrera observed that the two classrooms had soiled and stained carpets. LPA Herrera observed that the facility use the carpets for circle time. Furthermore, toilet #4 (biggest stall) did not have caps which exposed the metal bolts. Additionally toilet #4 did not flush properly, which posed a potential health, safety or personal rights to chidlren in care.
Based on interview and record review, the licensee did not comply with the section cited above. Staff 1 Center Director was not updated on licensing records. Director packet will need to be filed with CCLD. Director's employee file missing Proof of orientation, LIC500, LIC501, LIC610, LIC308, LIC309, Board of resolution letter. Licensing was last updated on the director in 2015 for Brooks Head start which poses a potential health, safety or personal rights risk to persons in care.
