Based on record review, the licensee did not comply with the section cited above in LPA observed that the "School Refrigerator" had rusty rails on the inside of the fridge, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record, the licensee did not comply with the section cited above in that LPA observed that Staff 7 did not have their fingerprints cleared or associated to the facility, which poses an immediate health, safety or personal rights risk to persons in care. A civil penalty of $100 was assessed on today's date.

Based on observation, the licensee did not comply with the section cited above in LPA observed in Main Preschool Office Mulit-Purpose room had various chemicals under the sink that included ant and roach killer, paint, Draino, cleaning sprays. LPA observed that children do have access to this area. The office is not an area that children learn, play, or stay. Yet, children and parents do have access to. which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in that LPA observed that the following staff had missing components to their files: Staff 2, Staff 3, Staff 4, Staff 5, Staff 6, and Staff 7. Which include immunziations, mandated reporter, and LIC documetns, which poses/posed a potential health, safety or personal rights risk to persons in care.
