During today's inspection, LPA observed two rooms that are separated by a door that the facility uses for daycare. The door remained open for the children to roam through both rooms. LPA observed five children, the Director and Staff #2 to be in one room at the same time for a short period of time, leaving the other three children left unattended in the other room. The children were not under supervision, including visual supervision, of a teacher at all times. Based on observation, the facility did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

During record review, the Director provided a document to LPA showing the last time a fire drill was conducted was on 06/25/2020. Director was unaware that fire drills need to be completed once every six months. Based on interview and record review, the facility did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

During today's inspection, LPA observed cots being stacked with children's bedding that is in contact with other children's bedding. Based on observation, the facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

During record review and interviews, it was discovered that Director's mandated reporter training expired on 05/15/2021 and it has not been renewed. Also, Staff #2 has not completed the training at all in the last two years. Staff #2 began employment at this daycare in August 2023. Based on interview and record review, the facility did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
