Based on observation, the licensee did not comply with the section cited above as the pool gate was not self-latching and self-closing as it was misaligned, which poses an immidiate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to repair the pool gate latch and notify CCLD Fresno of the completed repair by POC Date. If the latch cannot be repaired then Licensee stated that he will notify CCLD Fresno and provide a copy of the new pool gate purchase and will request a POC extension.

Based on record review, the licensees did not comply with the section cited above as Licensees had an expired Mandated Reporter Training which expired on 04/18/2025. Licensees did not renew Mandated reporter training as required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to renew the Mandated Reporter trainings and provide Community Care Licensing a copy of completion certificates by POC due date.

Based on record review, the licensees did not comply with the section cited above in Licensee pediatric 1st Aid/CPR trainings expired on 05/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will enroll in an EMSA certified training class and provide proof to Community Care Licensing by 09/23/2025.

During records review LPA Herrera did not observe any 15-minute sleep logs for child #1, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Travao stated that she will provide a 15-minute sleep log for child #1 which will document safe sleep checks for the next two weeks. The 15-minute sleep log will be submitted to CCLD Fresno via text/email by POC deadline.

Based on observation, the licensee did not comply with the section cited above in the fence surrounding the in-ground pool has a gap on the west side of the fence that is wider than 4 inches. The anchor at the top of the fencing was missing which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to secure the fencing and provide a photo of completed repair to Community Care Licensing.

Based on record review, the licensee did not comply with the section cited above in Licensee had initially completed the required Mandated Reporter Training which expired on 10/3/2019. Licensee did not renew Mandated reporter training as required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to attend part II of the training and provide Community Care Licensing a copy of completion certificate by POC due date.

Based on record review, the licensee did not comply with the section cited above in Licensee pediatric 1st Aid/CPR training expired on 7/11/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will enroll in an EMSA certified training class and provide proof to Community Care Licensing
