Based on observation, the licensee did not comply with the section cited above in that infant crib has a Dock-A-Tot for the infant to sleep in to keep him on his back which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the Dock-A-Tot pad. Going forward, cribs for infants must be clear of all loose articles and objects. There should be nothing in the crib, other than the infan. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on observation, the licensee did not comply with the section cited above in that crib had a blanket in the crib covering the child, a few toys and was sleeping in a Dock-A-Tot which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the blanket, toys and Dock-A-Tot pad. Going forward, cribs for infants must be clear of all loose articles and objects. There should be nothing in the crib, other than the infant. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above in that sleep logs were not being used for all infants up to age two which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Napping children must be checked every 15 minutes and document the date; infant’s name; time of each 15-minute check. These sleep logs must be maintained for all infants us to two years in age. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
