Based on record review, the licensee did not comply with the section cited above in the CPR and First Aid certificate provided was only for Adults not Pediatric which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA the correct certifcate that states Pediatric CPR and First aid to the LPA by email.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants only had 1 days worth of sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send a copy of sleep logs for each infant for 2 weeks to the LPA by email daily until POC date of 10/22/2024 to ensure they are being recorded each day.

Based on observation and interview record review, the licensee did not comply with the section cited above in one adult volunteer present in daycare which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have volunteer fingerprinted and send to LPA proof by POC date. Licensee's husband will replace volunteer to remain in ratio.

Based on record review, the licensee did not comply with the section cited above in 1 out of 8 children did not have form filled out prior to enrollment which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have parent fill out LIC282 and submit to LPA by POC date by end of business day.

Based on interview and record review, the licensee did not comply with the section cited above in not having sleep logs on file for 4 infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete one weeks worth of sleep logs and send to LPA by email by POC date before end of business day. Licensee will also review safe sleep regulations and send a statement on what she learned from refresher by email by end of business day to LPA email.

Based on record review, the licensee did not comply with the section cited above in 1 out of 8 children did not have immunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Complete blue card and submit to LPA by POC due date by end of business day.

Based on observation interview, the licensee did not comply with the section cited above in by having 3 baby walkers present in daycare which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all walkers and and daycare. Licensee will write a statement understanding that these items are not to be on premises for family daycares and send to LPA by email by POC date end of business day.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as licensee failed to have required forms maintained in her helper's file and was verified by her helper, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee would send corrections to LPA via email by 10/14/2022

Based on observation and record review, the licensee did not comply with the section cited above as 6 out of 6 children files are not properly documented on California immunization blue form, which poses a potential health, safety or personal rights risk to persons in care.
