Based on record review, both licensees did not comply with the section cited above in not renewing Mandated Reporter Training certificates, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure updating their mandated reporter "child care provider" training and submit copies of their certificates to LPA by due date of 08/18/2025. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on observation, the licensee did not comply with the section cited above in that child-proof latches have worn out and/or are not functioning properly which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees to purchase new latches and install them on the doors under the kitchen sink and drawer next to the stove (or move sharp utensils to an upper cabinet). Email pictures of new locks to LPA as proof of correction.

Based on observation, the licensee did not comply with the section cited above in that child-proof latches have worn out and/or are not functioning properly which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees to purchase new latches and install them on the doors under the kitchen sink and drawer next to the stove (or move knives to an upper cabinet).

Based on record review, the licensee did not comply with the section cited above in that last disaster drill was done on August 10, 2022, 9 months ago which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees to conduct a disaster drill (fire or earthquake) and going forward complete a drill at least once every six months.
