Resolution: Licensee will submit the completed LIC9227 for the infant to LPA by email by POC dated 3/26/2025.

Based on record review, the licensee did not comply with the section cited above 1 child has no immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email the immunization record for the child by POC dated 4/25/2025 to LPA by email.

Based on record review, the licensee did not comply with the section cited above all employees are missing flu shot or decline of flu shot and 1 employee is missing MMR immunization which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee provide MMR for the employee missing the immunization. License will provide proof of the 5 staff for the flu shot or signed decline of flu shot to LPA by email by the POC dated 4/25/2025.

Based on record review, the licensee did not comply with the section cited above Licensee has not consistantly completed the daily inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submitt all daily checks until 4/25/2025 at the end of the POC dated 4/25/2025 to LPA by email.

Based on record review, the licensee did not comply with the section cited above 2 employee's do not have a signed LIC9052 on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have the 2 employees sign the LIC9052 and submit to LPA by email by the POC dated 4/25/2025.

Based on record review, the licensee did not comply with the section cited above Licensee's Mandated Reporter expired 2/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will completed the Child Care Provider Mandated Reporter Training and send to LPA by email by POC dated 4/25/2025.

Based on observation, the licensee did not comply with the section cited above the lockable device for the pool gate is only 51 inches above the ground which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to make the backyard is to go off limits until the lockable device is moved at least 60 inches above the ground. The backyard can not go on limits until LPA can verify the lockable device has been moved higher by POC visit.

Based on observation, the licensee did not comply with the section cited above Licensee does not have the correct pool alarm which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to make the backyard is to go off limits until the in pool alarm has been installed in the pool. The backyard can not go on limits until LPA can verify the pool alarm is installed and functioning by POC visit.

Based on record review, the licensee did not comply with the section cited above in 2 children's files are missing a signed LIC9224 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide the licensing reports and have the LIC9224 signed by the parents of the children that are missing them to LPA by POC dated 4/25/2025

Based on record review, the licensee did not comply with the section cited above Licensee is not documenting every 15 minutes on sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send sleep logs conducted for the infant through 4/25/2025 to LPA by email by POC dated 4/25/2025.

Based on record review, the licensee did not comply with the section cited above in 1 infant is missing the LIC995A which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain the completed LIC995A for the infant with no file.

Based on observation, the licensee did not comply with the section cited above in 1 infant was sleeping in a crib with a flat sheet wrapped around mattress not a tight fitted sheet which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will buy a tight fitted sheet for the crib and replace the sheet that is currently in the crib.

Based on observation, the licensee did not comply with the section cited above in there was a toy in the crib with a sleeping infant which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed toy immediately and will review the Safe Sleep video on https://safetosleep.nichd.nih.gov/resources/videos titled Safe Infant Sleep for Grandparents and Other Trusted Caregivers. After watching Licensee will submit a statement on what she learned from the video and how she will maintain the safe sleep regulations.

Based on observation, the licensee did not comply with the section cited above a blanket was hanging over the side of the crib which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blanket hanging from the side of crib immediately and will review the Safe Sleep video on https://safetosleep.nichd.nih.gov/resources/videos titled Safe Infant Sleep for Grandparents and Other Trusted Caregivers. After watching Licensee will submit a statement on what she learned from the video and how she will maintain the safe sleep regulations.

Based on interview and record review, the licensee did not comply with the section cited above infant does not have sleep logs for infant in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will document the infants nap on a sleep log for 3/12/2024. Licensee will submit a statement on how she will maintain the sleep logs for infants.

Based on record review, the licensee did not comply with the section cited above in 1 infant does not have immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain the immunizations for the infant with no file.
