BENITEZ, ROSA AND MARITZA

Based on record review, the licensee did not comply with the section cited above in that licensee Maritza Benitez and helper Megan Rolfes all took the Mandated Reporter together but only received one certificate in licensee Rosa Benitez' name. Each person in the facility must have their own Mandated Reporter certificate (AB1207) and renewed every two years otherwise it poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mandated Reporter certificates must be renewed in each person's name and renewed every two years.

Based on record review, the licensee did not comply with the section cited above in that both licensee's CPR/1st Aid certificates expired 10/10/2022 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Rosa Benitez and Maritza Benitez must renew their CPR/1st Aid training and get new certificates. CPR and 1st Aid are required to be renewed every two years.

Based on record review, the licensee did not comply with the section cited above in that Licensee had infants under 12 months without maintaining an infant sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will maintain sleep logs going forward for all napping infants from age birth to 24 months.

Based on observation, the licensee did not comply with the section cited above in that Licensee had a bottle of cleaner in an unsecured cabinet under the kitchen sink labeled "keep out of reach of children" which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the cleaner and relocated it to a secure location.

Based on record review, the licensee did not comply with the section cited above in that Licensee had infants under 12 months of age without the LIC9227 Infant Safe Sleep Plan signed by parents in children's files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to collect parent's signatures on LIC9227 forms for all infants 12 months old or younger who are currently in care and for all future infants in care aged birth to 12 months.
