At 10:15AM., Based on record review, volunteer staff (S3) proof of immunization missing from the facility records. This poses a potential health and safety risk to children in care.

Resolution: Licensee will collected proof of immunization by volunteer by the due date: 6/24/2025. Proof of correction will be submitted to the Department via email.

At 10:30AM., Based on record review, LPA confirmed children’s files (C2, C4, and C7) missing from the facility records. This poses a potential health and safety risk to children in care.

Resolution: Licensee will collected required licensing forms, and information for each child in care by the due date: 6/24/2025. Proof of correction will be submitted to the Department via email.

At 10:45AM., Based on record review, LPA confirmed licensee’s is not maintaining infant napping logs documenting each 15 minute review. This poses a potential health and safety risk to children in care.

Resolution: Licensee will begin documenting each review of napping infants in care, by the due date: 6/24/2025. Log should inlcude: date, time, and child's name. Proof of correction will be submitted to the Department via email.
