At 2:40PM., Based on record review, interview, and observation, LPA confirmed licensee operating beyond limits stated on license with 12 children (5 infant-age) children in care. This poses an immediate risk to the health and safety of children in care.

Resolution: Licensee will dis-enroll infant-age child or change schedules to ensure compliance with capacity limits by the due date: 5/7/2025. Proof of correction will be submitted to the Department via email. Updated Children's Roster (LIC9040) will be submitted to the Department via email.

At 2:45PM., Based on record review, LPA confirmed LIC9227, Individual Infant Sleeping Plan missing from qualifying infant's file (C1). This poses potential risk to the health and safety of children in care.

Resolution: Licensee will ensure guardian completed the LIC9227, Individual infant sleeping plan for qualifying infant in care (C1). Proof of correction will be submitted to Department via email.

At 1:40AM., Based on observation and interview, LPA confirmed licensee operating out of ratio, with one adult (Licensee) present with 11 children in care. This poses a potential risk to the health and safety of children in care.

Resolution: Licensee will submit written plan ensuring proper ratio is met while facility capacity exceeds small capacity limits by due date: 5/12/2025. Proof of correction will be submitted to department via email.

Based on observation and interview with licenseee, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA discussed with licensee how to prevent the lack of supervision from happening. Licensee stated she will send a written plan on how to schedule staffing to keep from having supervision issues.
