Based on record review, the licensee did not comply with the section cited above in that S1 and S2's First Aid/CPR certificate expired on 09/09/25 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated S1 and S2 have an appointment on February 7th, 2026 to renew their First Aid/CPR certificate. Licensee agrees to complete the training and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before POC due date.

Based on interview, Licensee stated S3 moved into the home beginning of January 2026, but does not have a criminal record clearance on file which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S3 obtain a criminal record clearance. Licensee agrees to submit a written statement with their understanding of the above regulation and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date. An immediate civil penalty of $500.00 has been assessed.

During the tour of the home, LPA observed "baby powder", on the lower shelf of a diaper changing table. Additionally, near the changing mat there was "A+D ointment cream" which states to contact poison control if a child consumes it. LPA also observed multiple diaper rash creams in an area accessible to day care children. This was all in the "Family Room #2". Lastly, there was a blender in between the "Kitchen" and "Breakfast Nook", which was in a cart low enough and accessible to daycare children. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Immediately, licensee removed all items and made them inaccessible to day care children. The poison was removed and kept behind a lock.

Based on record review, the licensee did not comply with the section cited above in 1 out of 3 children. C1 did not have immunizations on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of immunizations for C1 to the department, no later than POC due date.

Based on record review, the licensee did not comply with the section cited above regarding 3 out of 3 people. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of Mandated reporter training for herself and her two assistants to licensing no later than POC due date.

Based on record review, the licensee did not comply with the section cited above. Immunizations were not complete and on file: proof of measles is missing, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of MMR (measles) immunization to licensing no later than POC due date.
