Based on record review, the licensee did not comply with the section cited above in that licensee was unable to provide documentation showing proof of 15 minute sleep checks for C1 and C6 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document proof of 15 minute sleep checks for C1 and C6 for a two week period and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that there is no rescue pole with a body hook and a minimum fixed length of 12 feet present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a rescue pole with a body hook and a minimum fixed length of 12 feet and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that there was no daily log documenting the drowning prevention safety features or safety equipment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document daily inspections on the drowning prevention safety features and safety equipment before arrival of the first daycare child and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S1-S3 does not have the Mandated Reporter Training (AB1207) completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1-S3 complete the Mandated Reporter Training (AB1207) and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S1-S3 did not have the Notice of Employee Rights (LIC9052) form on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1-S3 complete the Notice of Employee Rights (LIC9052) form and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S1-S3 do not have proof of influenza(Flu), pertussis (Tdap), measles (MMR), and Tuberculosis (TB) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1-S3 provide proof and/or obtain the required immunization(s) and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that there is no ASTM International Standard F2208 compliant pool alarm, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain an ASTM International Standard F2208 compliant pool alarm that, when placed in a swimming pool, will sound upon detecting an entrance into the water and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that there is no life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a life ring with a minimum exterior diameter of 17 inches that is approved by the United States Coast Guard and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review and licensees own admission, the section cited above was not in compliance in 2 out of 2 individuals. Mandated reporter training has not been completed for either "general" or for "child care providers". This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of Mandated Reporter Training (MRT) certificate for herself and her assistant no later than the POC due date. The licensee can complete the MRT online, at mandatedreporterca.com

Based on record review, the licensee did not comply with the section cited above in 1 out of 5 persons. 1 out of 5 children do not have immunizations on file, due to never getting them. There is a doctors note stating that the child did not get their immunizations on file, but this is not a temporary exemption. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee was given a copy of the Regulation, section 102418: Immunizations. Licensee agrees to read the regulations and submit future proof of plan for the child no later than the POC due date. This plan must include coming into compliance with the Title 22 Regulation given to licensee.

Based on record review, the licensee did not comply with the section cited above in 2 out of 5 persons. 2 out of 5 children did not have LIC995 signed by an authorized representative and on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licsensee agrees to submit proof of completed LIC995 for the 2 children missing the form, via text, email or mail, no later than POC due date.

Based on record review, the licensee did not comply with the section cited above. 15 minute sleep log was on file, however the last one was documented on 08/14/2023. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to immediately start implementing 15-minute sleep logs in her facility and submit proof of log to licensing no later than the POC due date.

The licensee could not provide proof of a current Fire Drill log. Last drill conducted on 05/28/2021. Based on LPAs observation, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee has agreed to conduct a Fire and Disaster drill and provide written documentation of the drill, no later then 03/10/2022.
