Based on record review, the licensee did not comply with the section cited above in 2 out of 2 persons. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the mandated reporter training for "Child Care Providers", and send proof of certificate for both herself and her assistant. This is due to the department no later than the POC due date.
Based on licensees admission, there were not documents to review regarding 15 minute checks for infants in care. Although licensee stated she has been visually observing infant(s) in care for the duration that that sleep, licensee has not been keeping a 15 minute sleep log on the infant(s) in care.This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of 15 minute sleep log(s) for any and all infants currently or newly enrolled from todays date to one day before the POC due date. Licensee agrees to submit proof of this documentation on POC due date.
