Based on record review, the licensee did not comply with the section cited above in that C1 was missing the Individual Infant Sleeping Plan [LIC 9227 (3/20)] which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the LIC9227 form for C1 and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S1 and S2 did not complete the mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the mandated reporter training (mandatedreporterca.com) for S1 and S2 and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S2 did not have a file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create and maintain a personnel record for S2 and include required licensing forms and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that C1 was missing immunization record which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain C1's immunization record and have a copy in the file and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that there was no roster available which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a roster with the information of all the daycare children enrolled and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review and licensees own admission, the licensee did not comply with the section cited above in 1 out of 2 persons. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant needs to show proof of Mandated Reporter Training (MRT) certificates for both, "general" and "Child Care Providers". These required training's can be accessed online at mandatedreporterca.com This is due to the department no later than the POC due date listed above.

Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain children's files for each child in care and keep in a folder for review for the future. Licensee agrees to show proof of LIC995 for C1 and C4 no later than the POC due date.

Based on record review and licensees own admission, the licensee did not comply with the section cited above in 2 out of 2 infants in care. There were no 15 minute checks on file for either infant. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of 15 minute checks for both infants in care no later than POC due date.

Based on record review and licensees own admission, the licensee did not comply with the section cited above. 2 out of 2 staff are missing immunizations. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of MMR for herself and proof of all immunizations including MMR (measles), Tdap (pertussis), TB (tuberculosis) and flu shot, or declination of flu shot for assistant. These are due to the department no later than the POC due date.

Based on record review, the licensee did not comply with the section cited above due to not being able to provide proof of a current CPR & First Aid card at time of visit, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee has agreed to provide proof of a current CPR & First Aid EMSA card or proof of enrollement no later than 09/05/2022. If taking the course again the licensee has agreed to submit the updated current card.
