Based on record review, the licensee did not comply with the section cited above, C3, C4 and C7 immunization records are missing from their file and are not available to review, which poses a potential health risk to persons in care.

Resolution: Licensee agrees to obtain and maintain the immunization records for C3, C4 and C7. Licensee agrees to provide a copy of the immunization record for C3, C4 and C7 to LPA via email by close of business on 08/22/2024.

Based on record review, the licensee did not comply with the section cited above. The licensee stated they have 25 children enrolled, the facility roster documents 43 children enrolled. The facility roster is not current at the time of the visit, which poses a potential safety risk to persons in care.

Resolution: Licensee agrees to update their facility roster to reflect the current status, maintain the facility roster, and provide a copy of the facility roster to LPA via email by close of business on 08/22/2024.

Based on record review, the licensee did not comply with the section cited above, C2, C5, C7, and C8 Infant Sleep Logs are not available for LPA to review during the visit, This poses a potential safety risk to persons in care.

Resolution: Licensee agrees to create an Infant Sleep Log for C2, C5, C7, and C8 that documents the date, infant’s name, and time of each 15-minute check, initials and provide a copy to LPA via email by close of business on 08/22/2024.
