Based on record review and licensee's own omission, the licensee did not comply with the section cited above, the last fire drill conducted and documented was on 12/14/2023, which poses a potential safety risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill with children in care by 06/28/2024, document the fire/disaster drill, complete fire/disaster drill at least once every six months and provide a copy of the documented fire/disaster drill to LPA via email by COB on 06/28/2024.

Based on record review, the licensee did not comply with the section cited above, S1 Mandated Reporter Child Care Providers (AB1207) Training was last completed on 03/21/2021 and S2 last completed on 03/23/2021, which poses a potential safety risk to persons in care.

Resolution: Licensee agrees to have S1 and S2 complete Mandated Reporter Child Care Providers (AB1207) Training and provide a copy of the Mandated Reporter Child Care Providers (AB1207) Training certificate of completion to LPA via email by COB on 07/10/2024. www.mandatedreporterca.com

Based on record review, the licensee did not comply with the section cited above, S2 does not have proof of Tuberculosis (TB) clearance which poses a potential health risk to persons in care.

Resolution: Licensee agrees to obtain proof of S2 Tuberculosis (TB) clearance and provide a copy to LPA via email by COB on 07/10/2024.

Based record review, the licensee did not comply with the section cited above, S1 and S2 Pediatric CPR/First Aid Certification completion date of 06/21/2023, expire date of 06/21/2025 was not completed with an EMSA approved vendor, which poses potential safety risk to persons in care.

Resolution: Licensee agrees to have S1 and S2 enroll in or complete Pediatric CPR/First Aid Certification issued by American Red Cross, American Heart Association, or by an Emergency Medical Services Authority (EMSA) approved vendor and provide proof of enrollment and/or completion to LPA via email by COB on 07/10/2024. www.emsa.ca.gov

Based on record review, the licensee did not comply with the section cited above, C1, C2, and C3 immunization record is not available for review during the visit, which poses a potential health risk to persons in care.

Resolution: Licensee agrees to obtain the immunization record for C1, C2 and C3 and provide a copy of the immunization record to LPA via email by COB on 07/10/2024.

Based on LPA observation and record review, the licensee did not comply with the section cited above, the infant sleep log for C4 throughout the month of June 2024, has the same preselected 15 minute sleep checks documented. During the visit, LPA did not observe C4 sleeping during the preselected times 9:15 a.m. - 10:00 a.m., which poses a potential safety risk to persons in care.

Resolution: Licensee agrees to create a infant sleep log for C4, that includes the date, infants name, and time of each 15 minute check, initials of staff whom physically checked infant, and provide a copy to LPA via email by COB on 07/10/2024.
