Based on record review, the licensee did not comply with the section cited above in that the last documented disaster drill was on 01/15/25 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct fire drills and disaster drills at least once every six months and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that C1 and C3's 15 minute sleep logs were not up to date and documented which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to physically check on the infants every 15 minutes while they are sleeping, document times and observations and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that the Licensee's pediatric cardiopulmonary resuscitation and pediatric first aid expired on 09/17/25, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to enroll and complete the pediatric cardiopulmonary resuscitation and pediatric first aid training and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above. 1 out of 2 children's immunizations were not kept on record. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of immunizatins for C1 no later than POC due date.
