Based on LPA observation and record review, the licensee did not comply with the section cited above in 5 out of 5 children's records reviewed. LIC282 Affidavit Regarding Liability Insurance was not present for review, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was made aware of this regulation and provided blank copies of LIC282 to add to all children's records. Based on observation and record review, the licensee did not comply with the section cited above. Licensee's First Aid/CPR certification has expired as of 03/06/2025 and is in need of renewal, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete/renew their First Aid/CPR Certrificate and provide proof of completion certificate to LPA on or before the stated POC date of 12/05/2025. Please complete EMSA, American Red Cross or American Heart Association certificate. Submissions can be made by text message, email, or US post sent directly to the Riverside Regional Office.
Based on observation, the licensee did not comply with the section cited above. Licensee did not have staircase properly barricaded or fenced. Children under 5 years of age were present and in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understand the staircase must be properly barricaded/fenced at all times when children under age 5 are present and in care. Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate pool alarm on or before the stated POC date of 12/05/2025.

Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a 12 foot rescue pole present with body hook, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate 12 foot rescue pole with body hook on or before the stated POC date of 12/05/2025.
Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a pool alarm nor an established log, which poses a potential health, safety or personal rights risk to persons in care
Resolution: Licensee agrees to meet the above noted regulation by acquiring a pool alarm and then establishing an log of its daily tests on or before the stated POC date of 12/05/2025.

Based on observation the licensee did not comply with the section cited above in count: licensee had a total of 8 children with no assistant present which poses an immediate health, safety or personal rights risk to persons in care. LPA observed 2 infants and 6 preschool children.
Resolution: The licensee has agreed to submit a written statement on capacity of acknowledgement, understanding and compliance to CCL by 11/22/24. The licensee was informed of the small daycare capacity when an assistant is not present.
Based on review, the licensee did not comply with the section cited above LPA observed licensee did not have infant log sheets for both infants present during inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will provide log sheets for children over one year old going forward. Licensee stated she will send a written statement to CCL of acknowledgement, understanding and compliance to this regulation by 11/28/24
Based on record review, the licensee did not comply with the section cited above. LPA observed during record review licensee did not have a current Mandated Reporter Training certificate which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. LPA observed childrens files were missing immunization records which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtain all children's immunization records and send a copy to CCL by 11/28/24.
Based on LPA's observation the licensee had children under age 5 in care on this date without the stairs being barricaded, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee is leaving for a 3 week vacation and has stated, a gate will be in place prior to taking children in care upon returning from vacation.
Based on LPA's observation upon arrival the licensee was present with 10 children. Per the licensee the licensees assistant couldn't come today and her husband had an appointment. The licensee did not comply with the section cited above in that she had a total of 10 children with no assistant present, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee has agreed to submit a written statement of her understanding of the regulation for staffing ratio and capacity. The licensee, has stated she will ensure that she has a qualified assistant present on the days she has 9 children plus.
