Based on observation and interview, the licensee did not comply with the section cited above in that there were 2 out of 3 infants present (Child 4 [C4] & Child 5 [C5]) that did not have 15 minute safe sleep logs completed. Additionally, Child 1 (C1)'s last 15 minute safe sleep log was completed in June of 2025 and has no other more recent 15 minute safe sleep logs available to review, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit proof (pictures) of 3 days of completed 15 minute safe sleep logs for C1, C4, and C5 to the Department by the POC due date.
