Resolution: Licensee was made aware of the above noted regulation. Licensee agrees to have LIC9227 completed and on file for C5 and/or all future enrolled children under the age of one years old. Licensee will submit proof of correction to LPA on or before the stated POC date of 02/10/2025. Submission can be made my email: justin.giese@dss.ca.gov
