Resolution: Licensee agrees to have C1-C4's authorized representative(s) complete the Affidavit Regarding Liability Insurance (LIC282) form and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S1 does not have the Mandated Reporter Training (AB1207) completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 complete the Mandated Reporter Training (AB1207) and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that C2 did not have the Notification of Parent's Rights (LIC995A) form on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C2's authorized representative(s) complete the Notification of Parent's Rights (LIC995A) form and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that there is no ASTM International Standard F2208 compliant pool alarm, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain an ASTM International Standard F2208 compliant pool alarm that, when placed in a swimming pool, will sound upon detecting an entrance into the water and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that there is no life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a life ring with a minimum exterior diameter of 17 inches that is approved by the United States Coast Guard and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that there is no rescue pole with a body hook and a minimum fixed length of 12 feet present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a rescue pole with a body hook and a minimum fixed length of 12 feet and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that there was no daily log documenting the drowning prevention safety features or safety equipment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document daily inspections on the drowning prevention safety features and safety equipment before arrival of the first daycare child and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review and licensees own admission, the licensee did not comply with the section cited above. The licensees did not have 15 minute safe sleep logs on file for the infants in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to submit proof of safe sleep logs for all infants in care no later than the POC due date. Infants up to 24 months of age must have a safe sleep log on file.

Based on record review, the licensee did not comply with the section cited above in 3 residents of the home. There were no TB tests on file for 3 persons. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to submit proof of TB clearances for 3 adults residing in the home no later than POC due date.
