Based on LPA's observation, interview, and record review, the licensee did not comply with the section cited above. Saff #1 did not have proof of Tuberculosis clearance on file, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that they could not provide proof of mandated reporter certificate for their assistant which posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in that they did not keep an LIC 282 on file for all children in which posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that they did not keep and LIC 9227 for a infant children in care which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof of completed LIC 9227 for all infant children under 12 mos. and send proof to LPA by POC due date of 11/15/2024

Based on observation and interview, the licensee did not comply with the section cited above in that they did not keep infant sleep log for all infants in care which posed a potential health, safety or personal rights risk to persons in care.

Based on observatin and interview, the licensee did not comply with the section cited above in that they did not have a working smoke detector during the annual inspection which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to purchase a new smoke dector and send LPA proof of purchase by the POC due date of 11/15/2024

Based on record review, the licensee did not comply with the section cited above in that C2, C4, and C7 did not have a 15 min log on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to begin documenting 15 min checks and submit log to CCL by 4/10/23.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current roster on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit completed roster to CCL by 4/10/23.

Based on observation, the licensee did not comply with the section cited above in that Licensee did not ensure cribs were not free of loose articles, two infants had blankets, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the blankets during the inspection. Licensee agrees to submit a written statement of their understatement of the safe sleep regulations and agreeing to maintain cribs free of articles.

Based on observation, the licensee did not comply with the section cited above in that when LPA arrived Licensee was out of ratio due to having 8 children in which none were school age, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During the inspection Licensees assistant arrived to help. Licensee agrees to maintain compliance and submit a written statement detailing how they will maintain ratios.

Based on record review, the licensee did not comply with the section cited above in that S1 did not have a mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that Licensee and S1 did not have a current CPR on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that S1 did not have their immunization's on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit immunization (MMR, Tdap, Flu - written statement needed if declining) for S1 and submit proof to CCL by 4/10/23.
