Based on observation, interview, and record review, the licensee did not comply with the section cited above as LPA observed an uncleared adult present in the home which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee had the adult get fingerprint taken while LPA was at the facility and provided receipt of live scan.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 1 staff did not have immunization record of Pertussis on file for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will get shot record of Pertussis or Tdap and other required documents and place it in the staff file. Licensee will email LPA an attestation statement that it is complete by POC due date.

Resolution: Licensee will obtain proof of immunizations for the assistant and email them to LPA by POC due date.

Resolution: Licensee will purchase a new Carbon Monoxide detector and send LPA a picture of it hanging up at the facility by POC due date.

Resolution: LPA provided the website Mandatedreporterca.com, so the assistant can obtain the Mandated Reporter certificate. LPA advised she must complete both the general training and the child care training. Licensee will obtain the certificates and email them to LPA by POC due date.
