Based on observation and interview, the licensee did not comply with the section cited above as LPA did not observe access gate that opens away from the swimming pool and is self-closing with a self-latching, key lockable device. Licensee stated she lost keys to the device but has been locked since, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get it fixed and will email LPA documentation by end of day POC due date. If licensee needs more time to fix it, Licensee will email or contact LPA for an extension. LPA will conduct a POC visit.
Based on observation, and interview, the licensee did not comply with the section cited above as LPA observed pool alarm working, but Licensee can't find documentation that pool alarms meets ASTM International Standard F2208 at time of LPA visit, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will find the documentation and email it to LPA by end of day POC due date. If Licensee needs more time, she will contact LPA for an extension.
Based on observation, the licensee did not comply with the section cited above as LPA observed the safety hook with a minimum length of 12 feet, but LPA did not observe it to be fixed, it is adjustable, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get it fixed and will email LPA documentation by end of day POC due date. If Licensee needs more time, she will contact LPA for an extension.
Based on LPAs observation the licensee did not comply with the section cited above and LPAs observed the firearms were locked in a safe with ammunition which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee purchased a second safe with LPAs present and stated she will lock the ammunition inside of it which will be separate from the firearms. Licensee shall submit pictures to LPA Ferrara by POC due date.
