Based on observation, the licensee did not comply with the section cited above as LPA observed the rescue pole with a body hook is not a fixed length, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get the rescue pole with a body hook with a fixed length of 12 feet and will email LPA by POC due date.
Based on observation and interview, the licensee did not comply with the section cited above as LPA observed an alarm in the in ground pool, but Licensee cannot find documentation that it is compliant with ASTM International Standard F2208. LPA did not observe the alarm working at time of LPA visit. [count] out of [total count] [(objects) (persons)] [identifiers] which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get an alarm that is compliant with ASTM F2208, and LPA observed Licensee ordered online.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed one assistant without criminal record clearance. Licensee stated assistant turned 18 recently. LPA record reviews showed assistant turned 18 in August, and has been employed at the facility since July 2024, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated assistant will get fingerprinted today. Licensee will email LPA receipt.
Based on observation, the licensee did not comply with the section cited above as LPA observed a life ring with minimum exterior diameter of 17 inches but not labeled United States Coast Guard, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will purchase the USCC approved life ring and will email LPA by POC due date.
