Based on interview, and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that staff #1 was missing the LIC 503 Health Screening Report. LPA also observed that staff #2 had a Health Screening Report on file but it was not signed by a qualified health screener/physician.

Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the outdoor inspection, LPA observed that a playground fixture (jeep with rubber tires) had exposed metal wires around the back tire. LPA also observed that the tire from the playground fixture was unscrewing from the frame and was very loose.

Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During record review, Licensing Program Analyst (LPA) spoke with facility representative who stated that he was unsure if the facility was tested for lead. Facility Representative stated that the children do not drink from the drinking fountains and only drink from bottled water.

Licensing Program Analyst (LPA) observed that there was one teacher supervising 13 napping children. However, not all of the children were napping as LPA observed several children walking around the classroom.
