Based on Child Records reviewed at 03:34pm, LPA observed that C1 was missing immunization records on file. The licensee did not comply with the section cited above for C1 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on facility record reviews, the licensee and S1 where missing AB 1207 Mandated Reporter Training certificates. Licensee and S1 did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she and S1 would ensure staff completed the AB 1207 Mandated Reported training module at mandatedreporterca.com or through Resource and Referral Agency and submit current certificate to the Derpartment by 03/01/24.
