Based on observation, interview, and record review, LPAs observed an in-ground pool with incomplete fencing providing possible access, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will add additional section of fencing to remove possible access to pool area by 10/03/25 and send photographic evidence to LPA via email.

Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons did not have vaccines on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain assistants vaccines and submit to LPA via email.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have herself and assistant complete the Mandated Reporter training and submit certificate to LPA by 5/26/23. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.
