ASHWORTH FAMILY CHILD CARE

Based on observation and interview, the licensee did not comply with the section cited above in 2 out of 3 infants in a play yard, where LPA observed personal items (blankets and pillow) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will write and sign a letter of understanding of Infant Safe sleep regulations CCR 102425 and send to LPA via email.

Based on observation and interview, LPA observed closed doors to an infant sleeping area without adult supervision inside, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review areas of infant sleep and staff lunch breaks, and write a plan to allow full visual supervision of infants at all times during times of sleeping. Licensee will send to LPA via email.

Based on record review, the licensee did not comply with the section cited above in 1 out of 6 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide LPA a copy of children under 2 years of age 15 minute sleep check documentation by email. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have her assistant and herself complete the Mandated Reporter Training and submit the certificate by email to LPA. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 persons which poses a potential health, safety or personal rights risk to persons in care.

Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Resolution: Licensee agrees to have obtain assistants immunizations (tdap & MMR & Flu) & TB clearance and submit to LPA by email. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in 4 out of 6 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain all children in her care updated immunizations and maintain them throughout their enrollment. Licensee agrees to submit proof of immunizations to LPA by email. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in 6 out of 6 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have all parents complete the LIC995A to place in childs file and submit proof to LPA by email. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Resolution: Licensee agrees to have parent complete LIC9227 for all children under the age of 1 years and update as needed. Licensee agrees to submit proof to LPA by email.
