Based on observation and record review children's record review began at 02:45PM completed at 03:57PM C1, C2, C3, C5, C6, and C7 were missing additional doses of immunizations. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to contact guardians of those children and request updated immunizations to update blue cdph 286. Licensee will have guardians bring or text the updated immunizations and can text or email the updated blue sheets to LPA Hernandez Torres.
