Based on record review, the facility did not comply with the section cited above and has not conducted a fire drills/disaster drill at least once within six months, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The facility will conduct a fire drill/disaster drill and will document when completed. Licensee will submit proof of competition to the department by 12/02/2025.
Based on LPAs’ observation, the licensee did not comply with the section cited above and was not present during LPAs’ inspection, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a facility schedule to ensure that Licensee’s temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. Licensee will submit this facility schedule to the department by 12/02/2025.
Based on LPA’s observation,the licensee did not comply with the section cited above, daycare children were observed in the off-limits garage. Additionally, interviews from Staff (S1 & S2) indicated that the facility utilizing the garage which is not approved on the Facility’s Fire Clearance and to be used for childcare which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a statement to ensure that the facility will not utilize the garage until an approved Fire Safety Clearance to use the area is obtained. The Statement will be submitted to the department by 11/05/2025.
