Based on interview and record review, two children (C5 and C6) did not have emergency information card on file. Both were missing all documents required to have on file. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee contacted guardians for both children and requested all forms required. Licensee agreed to take a picture of each document completed for both C5 and C6 and email, mail or scan the pictures to LPA Hernandez Torres on or before 04/11/2024.

Based on interview and record review, Licensee was not keeping sleep log for 22 month old infant in care. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee began to document sleep log for infant in care while LPA was present. Licensee placed a timer on her phone for 15 mins and would go check on infant and document the date, time, infants name and notes on sleep position. Licensee agreed to continue safe sleep log. LPA will call Licensee on or before 04/18/2024 and request the sleep log for a random date between 04/05-04/17 for licensee to email, mail or fax the requested sleep log.

Based on interview and record review, Five of the six children in care did not have files complete. Licensee reported their pet had ripped up all the files. LPA printed an entire packet for licensee to make copies of and place in a file. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have files for all children enrolled. LPA will return on or before 05/09/2023 to check for completion

Based on observation and record review, Licensee and assistant could not produce a valid EMSA pediatric CPR/1st aid training certification, it expired on 07/2021. Resolution: Licensee agreed to submit proof of registration for EMSA pediatric CPR/1st before 05/08/2023 for a class to be completed before June 8th 2023.

Resolution: Licensee agreed to have Sleep plan and Sleep Log for infant enrolled. LPA will return on or before 05/09/2023 to check for completion
