Based on interview and record review, the licensee was not able to produce LIC 9040 facility roster. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA printed out LIC 9040, licensee agreed to complete the LIC 9040 with all children currently enrolled and keep adding children as they enroll and documenting the date they dis-enroll. Licensee will email, mail or fax the LIC 9040 to LPA Hernandez Torres on or before 12/19/23.
