Based on LPA's staff record review, the Licensee was not able to furnish proof of negative Tuberculosis clearance for herself and S1. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee stated she will provide proof of negative Tuberculosis clearance for herself and S1 to the Department via email (jessica.gaumann@dss.ca.gov) by 12/8/25.

Based on LPA's record review, C1-C5, did not have documents verifying enrollment in school. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide documents verifying proof of school age children's enrollment in school to the Department via email (jessica.gaumann@dss.ca.gov) by 12/8/25.

Based on LPA's staff record review, Licesee was not able to furnish records for herself and S1. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of completed records for herself and S1 to include required immunizations, negative TB clearance, mandated reporter training and various required licesening forms to the Department via emial (jessica.gaumann@dss.ca.gov) by 12/8/25.

Based on LPA's staff record review, Licensee was not able to furnish proof of completed and current AB 1207 Mandated Reporter training for herself and S1. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of completion by visiting mandatedreporterca.com, and provided a current AB 1207 Mandated Reporter training for herself and S1 to the Department via email (jessica.gaumann@dss.ca.gov) by 12/8/25.

Based on licensee stating she was not able to furnish proof that an emergency drill had been conducted within the past six months. The Licensee violated section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of completed emergency drill to the Department via email (jessica.gaumann@dss.ca.gov) by 12/08/2025.

Based on LPA's observation of the 610A not being posted. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided blank 610A and Licesee compeleted form and provided evidence of it being posted to the Department via email (jessica.gaumann@dss.ca.gov) by 12/8/25.

Based on record review, At 09:24AM seven children's records were reviewed, C1, C2, C3 had LIC 282 Liability insuarance affidavit form incomplete. C1, C2, C5,C6 all had LIC 627 Consent for medical treatment missing and/or incomplete. LPA showed Licensee which children they were to have guardians complete the forms. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have the forms completed by Monday 09/19/2022 and furnish proof of completion to LPA Hernandez Torres by email, mail or fax at; Elpidia.hernandez-torres@dss.ca.gov, 1450 Neotomas Ave Suite 100, Santa Rosa, CA 95405

Based on record review, - At 09:24AM seven children's records were reviewed. Infant C7 was missing LIC 9227 Individual infant Sleeping plan. LPA showed Licensee which Infant it was to have guardians complete the forms. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
