Based on interview, record review, all five children were missing their blue CDPH 286. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA printed out Blue CDPH 286 and reviewed it with licensee. Licensee began to fill out BLue CDPH 286 for children in care. Licensee agreed to submit a completed CDPH 286 to LPA Hernandez Torres on or before 02/07/2024 via email, mail or fax.
Based on interview and record review, Two infants in care did not have a sleep log. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA printed out the Safe sleep regulations and reviewed them with licensee. Licensee began to keep Sleep log for infants in care as the inspection continued. Licensee agreed to submit copy of sleep log to LPA Hernandez Torres on or before 02/07/2024.
