Resolution: Licensee stated she will subtmit documentation of updated CHPD 286 for C1, C2, C3 and C5 to the Department by email to LPA Gaumann by 03/13/26 via email: jessica.gaumann@dss.ca.gov
Based on interview and record review, the licensee did not comply with the section cited above in that she was not able to produce a current roster of children in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA printed LIC 9040 Facility Roster and gave to Licensee. Licensee stated she will subtmit a current roster of children in care to the Department by email to LPA Gaumann by 03/13/26 via email: jessica.gaumann@dss.ca.gov
Based on observation and interview Fire extiguisher was in the red indicating it was overcharged. It was last serviced in December of 2022. the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would go purchase a new fire extinguisher as soon as children left amd send proof of purchase to LPA Hernandez Torres
Based on record review infant in care C4 was missing Sleep Log. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit the Sleep log for C4 from the week of 03/27-03/31 via email to LPA Hernandez Torres on or before 04/06/2023.
