Based on observation, interview, record review, the licensee was not documenting sleep logs for 23 month old infant in care. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to begin documenting sleep logs for infant in care. LPA will return to review sleep log on or after 06/12/2025.

Based on observation, Upon at arrival during today's at 07:50am inspection LPA's smelled urine in the home. Smell was in playroom. While in the play room LPAs observed a puppy pad for dogs to use the bathroom. The Licensee cleaned it up. LPA observed Skat on the carpet where the pad was cleaned up. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have the carpets cleaned professionally before 06/12/25.

Based on record review, 4 out of 5 children's records reviewed did not have immunizations transcribed on CDPH 286. One child did not have immunizations up to date. The licensee did not comply with the section cited above which poses posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to transcribe immunizations on CDPH 286. LPA Castro provided license with a copy of the form. LPA will return to review children records.

Based on observation,interview and record review, The roster shows 7 children enrolled. The 5 children that were present were not added on the roster. LPA gave Licensee the oppurtunity to add the children to have an accurate roster. Licensee reported it would take time away from provding services to children and would add them at a later time. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to update LIC 9040 before 06/12/25. LPA will return and review LIC 9040.

Based on observatio, interview, and record review, during record review 5 children's records were reviewed, and 4 children had incomplete files, with incomplete LIC 700, missing LIC 995A, missing LIC 627. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return on or after 06/12/25 to condcut a record review of the same children to confirm all children have complete files.

Based on interview and record review, Licensee was not able to produce a valid copy of AB 1207 Mandated Reporter training certificate. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee reported she would 45 days to complete the training and submit the certifcate to LPA Hernandez Torres by Close of busness day on 12/08/23 or before that date via email, mail or fax. https://mandatedreporterca.com/

Based on chilren's (C1-C7) records reviewed at 08:37AM which revealed LIC 700 was incomplete for C5 and C6. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would take a picture of the completed Blue CDPH 286 for both C5 and C6, and mail it in to LPA Elpidia Hernandez Torres at 1450 Neotomas Ave Suite 100, Santa Rosa CA 95405.

Based on licensee's statement which indicated a fire drill was conducted in May 2022, however; licensee did not furnish evidence to proof a disaster drill had been conducted in the last 6 months. A previous licensing report was issued on 08/31/2021 giving notice of the same violation, and as such a $250 civil penalty was assessed for a repeat violation. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would conduct a drill and submit proof on or before 08/19/2022 along with LIC 9098 to LPA Elpidia Hernandez Torres via mail, email or fax at; elpidia.hernandez-torres@dss.ca.gov, 1450 Neotomas Ave Suite 100, Santa Rosa CA, 95405, (707) 588-5099

Based on children's (C1-C7) records reviewed at 08:37AM which revealed C5 and C6 did not have immunizations transcribed on CDPH 286. Both children are not enrolled and attending school.
