Based on observation, While conducting tour of the home's back yard LPA observed the fence on the side of the yard that is need of repair it is missing fence pannels. On the other side of the yard there are items suchs as paint buckets and other rubbish that children can get a hold of. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to clean up the side of the yard and repair the fence missing pannals as that side is the emergency exit from the back yard. Licensee agreed to build a barrier on the opposite side to prevent the children from having acceses to anything hazardous.

Based on observation the licensee has a child safty gate that is not secured on the strairs with children under 5 years old in care. the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to purchase and install a new child gate and secure it the posts of the stairs, then take a picture and submit to LPA Hernandez Torres via email, mail or Fax.

Based on record review, Licensee was conducting 15 minuite checks but was not documenting the exact time of each check. the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA Printed Safe Sleep regulations and reviewed them with the licensee and provdied an example of the safe sleep log. Licensee agreed to submit a copy of the safe sleep log for both infants via email, mail or fax.
