Based on interview and record review, assistant was not able to provide proof of a disaster drill conducted with in the last 6 months. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The facility representative agreed to conduct a disaster drill on or before 01/20/24 and email, mail or fax proof of completion to LPA Hernandez Torres.

Based on interview and record review, the licensee, nor assistants were able to produce a valid AB 1207 mandated reporter training certificate. Both licensee, and assistant's AB 1207 expired March 2024. the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The facility representative agreed to complete AB 1207 Mandated reporter training certificate on or before 01/20/25 and email, mail or fax proof of completion to LPA Hernandez Torres.

Based on interview and record review, the licensee was not keeping a sleep log for infant in care. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA printed out an example of a sleep log. Licensee agreed to begin keeping a sleep log for infant in care.

Based on interview and record review, Licensee's assistant was not able to produce AB 1207 mandated reporter training certificate. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have assistant complete AB107 mandated reporter training and email, mail or fax certificate to LPA Hernandez Torres.

Based on interview and record review, two children in care did not have LIC 700 on file, or other required documents. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA printed out list of items required to be completed in each child's file. Licensee agreed to complete a file for those children and submit proof to LPA Hernandez Torres via email, mail or fax.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee's AB 1207 Training Certificate expired on 07/30/2021, Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would call those parents to receive an updated immunization record to update blue CDPH 286 form. Licensee will submit a picture as proof those children missing doses of vaccinations have recieved them.
