Based on records reviewed all children's files were missing a copy of admission agreement., the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interviews with staff (LS, S1-S3) and parent (P1) conducted between 10/20/25 and 12/01/25, it has been determined that facility staff failed to provide adequate supervision resulting in C1 choking C2, which poses a potential health, safety or personal rights risk to persons in care.
Based on interview and record review Facility transitioned to electronic record keeping and could not produce copy of admissions agreement. The center did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Based on records reviewed there were more than five instances where guardians were not signing children in and/or out. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, staff ( S1, S2, S5, S12) were missing evidence of negative TB results. During record review S1 and S12 were missing LIC 503 health screening report. Based on interviews conducted two children left the play yard unsupervised. This poses an immediate health and safety risk to children in care.
