Based on interview and record review, Licensee was not documenting 15 minute checks for infant in care. LPA printed out regulation and example of 15 minute check for licensee to include the infants name, date, time of each check and obervations made. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit sleep log for infant C5 for the day of 04/11/2023 via email, mail or fax.

Based on record review Licensee reported she does not carry liability insurance for the daycare and she is not required to. LPA agreed with licensee and reminded licensee although it's not required to obtain liability insurance, you are required to have LIC 282 signed and on file for each child's record. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA printed the LIC 282 for licensee. Licensee agreed to make copies of the LIC 282 and give to each child's guardian to sign and place in the child's file. Licensee agreed to submit self certification LIC 9098 documneting she has received and placed signed LIC 9098 in children's file. Licensee will email, mail or fac LIC 9098 to LPA Hernandez Torres on or before 04/20/2023

Based on record review and interview, Eight children were present, all were under 6 years old, one child was enrolled and attending T-K. Licensee reported she usually has an assistant present, how-ever assistant was on vacation currently and she believed she could have eight children present if they were under five years old. LPA printed out regulation and reviewed it with licensee. LPA also gave licensee a ratio hand out illustrating the regulation. the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit self certification with staament indicating what she is going to do to stay in compliance with the regualtion. Licensee will submit statment with LIC 9098 via email, mail or fax to LPA on or before 04/20/2023
