Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return to verify the necessary item (pole) is in place.

Based on record review, the licensee did not comply with the section cited above, 4 children did not have full vaccination records on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return on or before the POC date above to verifty the records of children who did not have their full vaccinations on file.

Based on observation the licensee did not comply with the section cited above which poses safety or personal rights risk to persons in care.

Resolution: LPA will return to verify the pool alarm was installed on or before POC date.

Based on observation the licensee did not comply with the section cited above, as there was no life ring present. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return to verify that licensee has a life ring in place.

Based on record review, the licensee did not comply with the section cited above in that fire drills were not conducted or documentated based on licensee statement which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to conduct and document fire drills once every six months & email a copy of the fire drill by POC due date.

Based on record review, the licensee did not comply with the section cited above in that the licensee is not documentating 15 minute sleep checks on all infants under the age of 2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to conduct and document infant sleep logs every 15 minutes & email a copy by POC due date to LPA L. Habtom.

Based on record review, the licensee did not comply with the section cited above in that 2 children had no files or paper work which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to receive the missing files for the children and & email a copy to LPA L. Habtom by POC due date

Based on record review, the licensee did not comply with the section cited above in that a few children files were missing immunizations which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that two play yards had mattresses longer than the play yard mattress size and were rolled under causing the top part to be elevated which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will replace the mattresses to the original size mattresses by the POC date.

Based on observation, the licensee did not comply with the section cited above in that there were 6 infants in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA L. Habtom will clear POC with a return visit.

Based on observation, the licensee did not comply with the section cited above in that there were 13 children but none were school age children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: One child was picked up while LPA L. Habtom was present bringing the census to 12.

Based on record review the licensee did not comply with the section cited above in one assistant, Elani, had a criminal record clearance, however she was not associated to the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states the employee has a clearance (LPA observed a copy) from another day care and they wanted her transferred to Galin's day care. They could not process the transfer through the Guardian system stating it was too confusing. She also called the Licensing Department and the office staff said they would complete it and did not. LPA reminded that the employee cannot be present in the facility until the transfer or fingerprint clearance has been processed. The employee shall be transferred to the facility by POC date of 4/6/22.
