Based on observation, interview and record review, the licensee did not comply with the section cited above in 1/4 adult residents do not have a criminal record clearance or an exemption from disqualification, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of livescan receipt and updated LIC 200 to CCL by POC, and submit proof of cleared and associated person to license, once processed/cleared. In addition, Licensee agrees to ensure uncleared adult resident is not in on limits daycare area nor around around daycare children during operational hours until a clearance is obtained and submitted to CCL.
Based on observation, the licensee did not comply with the section in that there were several play house equipment located outside that were broken and not in its original condition. There were also multiple webs and spiders located on the play ground set. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will remove the broken play house equipment and clean all playgound set from webs and spiders.
Based on observation, the licensee did not comply with the section cited above by having medication and industrial strength adhesive glue in the living room and having cleaning solutions outside accessible to children in care. The bathroom cabinet lock was broken and children had access to cleaning solutions.This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will replace lock on bathroom cabinet. Licensee removed medicine/vitamins and adhesive glue from living room. Licensee removed cleaning solutions that were located outside accessbile to children to a higher location while LPA was onsite.
Based on interview and record review, the licensee did not comply with the section cited above by not documenting emergency drills every 6 months which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send proof that she is completing drills every day for the next week and will ensure to complete every 6 months.
Based on interview and record review, the licensee did not comply with the section cited above in 2 of 2 infants in care under age of 12 months did not have the LIC9227 on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send proof to LPA that the LIC9227 has been completed for both infants in care.
Based on interveiw and record review, the licensee did not comply with the section cited above in 3 of 3 infants in care did not have documentation of 15 minute checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send a weeks worth of documentation of the 15 minute check for each infant in care.

Resolution: Licensee will ensure all infants have a sleeping plan in place in their file. The Licensee will submit proof of sleeping plan for Child #1 by POC due date to LPA.
Based on interview and record review, the licensee did not comply with the section cited above by not having a complete personnel file for Staff #2 and herself which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee will complete Staff #2's file and send proof to LPA by POC due date.

Resolution: Licensee will send LPA a picture of medication removed and locked along with proof of garage door locked by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above by not conducting fire and disaster drills, and not documenting them which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will conduct their first fire and disaster drill by POC due date, document it and send proof to LPA by POC due date. Licensee will ensure they are conduted atleast every 6 months.
Based on interview and record review, the licensee did not comply with the section cited above by not having documentation of sleeping log for Child #1 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure all infants are checked every 15 minutes and document it. Licensee will start logging Child #1's sleeping and will send proof to LPA by POC due date.
