Based on interview and record review, the licensee did not comply with the section cited above in both Licensee's do not have pediatric CPR & First Aid which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated they will both sign up for the correct CPR course and submit proof of completion via email to LPA.

Based on observation, the licensee did not comply with the section cited above with kitchen knives in unlocked drawer accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will install child locks on drawer and cabinets containing hazardousd items and will provide photographic evidence to LPA.

Based on observation, the licensee did not comply with the section cited above with a fire extinguisher not meeting the State Fire Marshall standard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a fully charged fire extinguisher (2A:10BC) and provide LPA with a photograph of fire extinguisher in place in facility.

Based on record review, the licensee did not comply with the section cited above, missing a sleep log in one out of one child's files, potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create and maintain a sleep log for each child under 24 months. Licensee will provide a week's worth of sleep log for C1 to LPA via email.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain immunization records and maintain a copy of them in the childs folders. Proof will be provided LPA via text message.

Resolution: Licensee stated she will have parents sign LIC 627 forms and will provide proof to LPA via text message. This form must remain in the childs folder.

Resolution: Licensee will remove all hazadorus cleaning solutions from hallway and counter tops making them inaccesible to children in care. Proof will be provided to LPA via text message.

Based on reord review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will take the Mandated Reporter course and will provide proof of the completed course to LPA via text message.

Based on record review, the licensee did not comply with the section cited above. Licensee does not have current First Aid/CPR certification on file which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete First Aid/CPR training and provide proof of completion to LPA by submitting a picture of certification via email or text on or before POC due date.

Based on record review the licensee did not comply with the section cited above. Licensee does not have proof of immunizations on file iwhich poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide copies of immunization records to LPA via text or email on or before POC due date.

Based on observation, the licensee did not comply with the section cited above. There were several safety hazards in backyard where play equipment located. Lighter fluid by back sliding door, two sheds unlocked with doors open and accessible to children, yard equipment on the side of one of the sheds. There was also a circular saw in a chair outside the sliding glass door. Licensee stated children were outside yesterday.which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide pictures of closed and locked sheds, gardening tools, saw, lighter fluid put away and made inaccessible to children in care. Licensee will provide proof of correction by submitting pictures of corrections via text message

Based on observation, the licensee did not comply with the section cited above. Bottle of cleaning solution (Lavender All Purpose Cleaner) on kitchen floor with seal broken which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide picture of cleaning product in locked cabinet under kitchen sink or in garage inaccessible to children in care. Licensee will provide proof of correction by submitting pictures of corrections via text message

Based on record review, the licensee did not comply with the section cited above. Licensee has not conducted a fire or earthquake drill at the facility since October 5, 2015 which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and earthquake drill, update log and provide proof to LPA via email or text. Licensee will provide a picture of the log and a video of the drills on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above. LIcensee has not completed mandated reporter training which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the online training for mandated reporter at www.mandatedreporterca.com and provide LPA with a picture of the certificate of completion for the General and Child Care Provider trainings on or before the POC due date 07/14/2022.
