Based on observation, the licensee did not comply with the section cited above in that one infant in care had a sleep sack along with a bear blankie which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed the sleep sack and bear blankie from infant in care. Licensee stated she will review safe sleep regulations and send email confirming she has read and understand the safe sleep regulations.
Based on observation and record review, the licensee did not comply with the section cited above in that 4 of 4 infants in care did not have the documentation of every 15 minute check which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send LPA proof of a weeks worth of infant sleep logs for the 4 infants in care. Licensee will email the proof to LPA.
Based on record review, the licensee did not comply with the section cited above by not having a sleeping plan or sleeping log for Child #1, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall create a sleeping plan and log for Child #1 and any other new infants prior to admitting them. Sleeping plan and log will be send to LPA via email by POC due date.
