Based on record review, the licensee did not comply with the section cited above in one out of three files, did not have current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Provider will have S3 complete General and Child Care Provider mandated reporter training on or before POC date of 08/01/2022. Provider will submit proof of completion by emailing LPA a copy of the certificates of completion via email.
Based on record review, the licensee did not comply with the section cited above in two out of children present, C7 and C8 were not listed on the current roster which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Provider updated roster during visit and will ensure in the future all children enrolled at the facility are listed on the roster.
